As the recovery phase from the COVID 19 pandemic continues, the ACERS community heart failure nursing service has resumed face to face clinic appointments, although this is still in a reduced capacity. Clinics run at St Leonards Hospital and some GP practices.  Home visits are offered for housebound patients.

Patients are prioritised according to clinical need, hence we are mainly seeing patients at risk of heart failure decompensation, recently discharged following a heart failure admission, or newly diagnosed requiring timely uptitration of medications.  We currently have a wait list for routine appointments.  Telephone and video link reviews via Attend Anywhere are offered where appropriate.

The IV diuretics/rapid response service continues to operate in a full capacity since the start of the pilot in September 2020.   Please refer to the pathway and send referrals to: huh-tr.homertonivdiuretics@nhs.net
