This is a monitoring service designed to allow GP’s to care for patients who have been discharged to them via the Enhanced Discharge Pathway who have SpO2 readings of between 91% - 92%. Patients who have been discharged on the Enhanced Discharge Pathway of SpO2 readings of 93% or higher and need regular monitoring should be referred to the Oximetry @home service.

Results will be provided within 2 hours of making the referral

If the technician takes a reading of 90% or below they will escalate to 999 and inform Homerton Hospital. All readings in the OOH period will be reviewed by the GP OOH service automatically. GP’s do not need to make any additional/separate referrals to GP OOH.


Eligibility criteria

Inclusions

  • Patient is registered with a C&H GP practice and currently physically resident in or within 0.5miles of the C&H borough boundary
  • Patient has been discharged to you on the Enhanced Discharge Pathway from a secondary care facility
  • Patients discharged with SpO2 of 91% - 92%
