From 1st December 2021, St Joseph’s Hospice will be providing a bereavement support for children and young people (CYP) who have lost a family member, care giver or other significant person
 
This is available free of charge for CYP up to the age of 18 who either live in City & Hackney or are registered with a City & Hackney GP. Support may include 1-2-1 and family sessions, parenting support, groups, art therapy and memorial events, and will be delivered virtually or face-to-face at the hospice. Interpreters are available as required.


Eligibility criteria

Inclusions

•    Children and young people up to the age of 18, their parent or caregiver
•    Registered with a City & Hackney GP or a resident of City & Hackney and have experienced a bereavement of  a significant person in their life 
 

 

Exclusions

•   Not a resident of City & Hackney or registered with a City & Hackney GP
•   Significant co-morbid mental health needs or an unresolved safeguarding concern at point of referral


How to refer

The following clinicians can refer to this service: GPs, Social or health care professional on behalf of the child or young person

Self-referral

You can self-refer for an assessment via the First Contact team on 0300 303 0400 or ask a GP or other professional to complete a referral on your behalf.
