The service provides:
•    Specialist multidisciplinary weight management programme 
 


Eligibility criteria

Inclusions

BMI ≥30 with Type 2 Diabetes or BMI ≥ 35 plus comorbidities* or BMI ≥ 40
And
•    Insufficient change from weight management intervention in the last 2 years (achieving/maintaining weight loss)
•    Assessed by C&H SAWMS as committed to attend the program
•    Registered with and referred by a City & Hackney GP
•    Over 18 years of age
 

*These include: HTN, DM, PCOS, dyslipidaemia, hyperinsulinaemia, asthma, COPD, OSA, OA, antipsychotics

Exclusions

•    Pregnancy 
•    Uncontrolled cardiac conditions including poorly controlled blood pressure 
•    Unable to access the community i.e. housebound
•    Unstable or severe mental illness including active eating disorder 
•    Active substance misuse
•    Patients who have undergone bariatric surgery in the past 24 months
•    Patients with moderate to severe learning disabilities
