Oviva Diabetes Support is a fully remote, digitally enabled structured education and behaviour change intervention for people living with type 2 diabetes. Oviva Diabetes Support offers personalised support led by expert healthcare professionals.

Type 2 Diabetes Structured Education – Digital Offer 

An exciting new structured education offer for our patients with Type 2 Diabetes is now available. Our Long Term Conditions Clinical Lead, Dr Shaine Mehta says;

“We are excited to introduce a new option for structured education for patients with Type 2 Diabetes - this offer sits alongside existing face to face structured education sessions run by Hackney Diabetes Centre. The new offer of support is provided by Oviva and is 100% remote. It may suit those patients that are working and a key benefit for us locally is that health coaching is available in multiple languages. There is evening and weekend provision, as well as self-directed education online or via an app.

Remember patients with new diagnosis or those that have missed key indicator targets and are at increased risk of complications would benefit from referral to supported program like this. Patients reportedly lose on average 4kg on completion of the program and 1 in 4 achieve remission of their diabetes.”

Support is available in the following languages: Arabic, Bengali (Shudo), Bengali (Sylheti), Cantonese, Dutch, Estonian, French, German, Greek, Gujarati, Hindi, Hungarian, Italian, Japanese, Lithuanian, Mandarin, Nepalese, Polish, Punjabi, Romanian, Spanish, Sylheti, Tamil, Turkish, Urdu.


Eligibility criteria

Inclusions

Be registered with a GP practice within the North East London CCG
Be aged 18 or over
Have a confirmed diagnosis of type 2 diabetes

Exclusions

People who are not registered with a GP practice within the North East London CCG
People under the age of 18
People with type 1 diabetes
People who are at risk of diabetes but do not have a recorded diagnosis of type 2 diabetes
People who do not have a recorded diagnosis of type 2 diabetes
Pregnant women
People with a significant learning disability or untreated mental health condition that makes it
difficult for them to engage in care
People who had previous bariatric surgery less than 2 years ago or have planned bariatric
surgery which will occur during the course of the programme
People who are already seeing a dietitian elsewhere
