The Community ENT service provided by Communitas aims to offer patients an appointment within four weeks for:

• Routine assessment and diagnosis 
• Routine treatment
• Follow up where necessary/clinically required

Eligibility criteria

Inclusions

ENT specialist care where surgery is an unlikely outcome post-consultation.  The full referral criteria can be found in the downloads section.

Routine and Urgent ENT

• All patients from 5 years refer to community ENT service (provided by Communitas).  The community service will clinically review all GP referrals within 72 hours and determine the appropriate pathway.  If it is evident during triage that a patient needs secondary care ENT, Communitas will make the onward referral to secondary care.
• All patients under 5 years refer directly to secondary care. 
• All patients of any age who require a 2WW referral refer to secondary care.

Emergency ENT

• For emergency care for patients under 16 years send letter via email to bartshealth.paediatricentpcs@nhs.net or contact on-call doctor via switchboard
• For emergency care for all patients under 16 years upwards refer via e-RS to the ENT Emergency RAS clinic at Royal London (Service ID 7944253)

Exclusions

• Patients who meet the criteria for AQP hearing assessments
• Red flags and sudden hearing loss 
• multiple and/or complex co-morbidities that require a multidisciplinary service across specialties
• already had a complete work-up and treatment options have already been tried in the community
