Key information on Falls

OTAGO Home Exercise Programme Pilot  

• Evidence based programme to reduce risk of falls in frail elderly patients

• Previously commissioned by LBH and provided by MRS independent Living but funding discontinued in 2015 -> gap in falls prevention for vulnerable cohort

• Twelve month pilot started June 2019 (MRS provision) -> evaluation will inform longer term commissioning decision (announced in June GP Bulletin)

• Referrals accepted from all health and social care practitioners -> keen to encourage GPs to consider referral (referral form to be emailed to service)

Falls Care Task and Finish Group – request for volunteers / interest

Objective: To review and agree role of primary care in

• Identifying those at risk of falls

• Managing assessment and intervention for those at risk of falls- in different health settings

• Agreed standard assessment -> appropriate intervention (to include bone health assessment and treatment)

• Agreed recording (to enable monitoring) and development of IT/EMIS to support this
