Eligibility criteria

Inclusions

Who’s Eligible?

Your patient is eligible if:

  • aged 18 or over.​​
  • Has ‘non-diabetic hyperglycaemia’ (NDH) identified by blood test within the last 12 months.​​
    • HbA1c of 42.0-47.9mmol/mol (6.0%-6.4%),​​
    • Fasting Plasma Glucose (FPG) of 5.5-6.9mmol/l,​​
  • If the patient has a history of Gestational Diabetes Mellitus (GDM) no blood test required

Exclusions

  • Pregnancy
  • Has blood results suggesting Type 2 Diabetes
  • Anyone aged over 80 without prior consideration for the risks and benefits of a programme, likely to result in weight loss for those individuals.
  • Bariatric Surgery within the last 2 years
  • Active Eating Disorder

Referral methods: EMIS managed referral

Once NDPP Programme Provider (Thrive Tribe) receive the referral they will contact the patient directly and start the enrolment process.  These programmes will be delivered face-to-face, digitally and remotely.
