Eligibility criteria

Inclusions

Hackney residents over the age of 18 with a suspected or diagnosed learning disability.

This will be as a result of their assessed IQ and adaptive functioning levels.

The Integrated  Learning Disabilities Service will assess whether an adult has a learning disability and what support they are entitled to, based on an assessment against the eligibility criteria.

How to refer

The following clinicians can refer to this service: GPs, Nurses, GPs, statutory and voluntary organisations, family and carers

Referral methods: Email

Referrals are accepted from GPs, statutory and voluntary organisations, family and carers but the person being referred must be aware of the referral.

Referrals should be made on a joint referral form

Self-referral

Self-referrals are also permitted.
