An individual funding request can still be made if treatment is denied under the EBI policy due to it being judged to have limited evidence base of clinical value.

NOTE:   GPs are responsible for obtaining prior approval if referring for the following procedures:

  • Tonsillectomy
  • Hernia
  • Chalazia
