We invite referrals to our fully-supported, 1-to-1 specialist exercise service for patients aged 18+ with long-term, complex health conditions or disabilities. This programme is designed to improve health, promote independence, and enhance overall well-being through safe, progressive exercise, tailored to each individual’s needs.
Programme Overview:
Who is it for?
This 12-week programme, with an option to extend for an additional 12 weeks, is open to patients aged 18+ who have long-term, complex health conditions or disabilities. It’s available to those living in or registered with a GP in the City of London or Hackney area.
What does it involve?
Participants will receive 1-to-1 specialist support, guided by trained professionals at Ability Bow gym. The programme focuses on integrating accessible exercise into participants' lives, promoting long-term health improvements and greater independence.
Programme length:
The initial programme lasts 12 weeks, with the option to extend for a further 12 weeks depending on progress and individual needs.
