An introduction or refresher of NHS Continuing Healthcare and NHS Funded Nursing care which will include:-

Key for GP’s is an understanding of the fast track process that can avoid unnecessary admissions for patients with a rapidly deteriorating condition that  may be entering the terminal phase by securing them out of hospital support at home.

There will be opportunity for you to ask questions.
