Community mental health services for people with moderate to severe mental health difficulties and complex life situations.
The client journey focuses on relationships between mental health, physical health, social networks and daily living.
There are also open access events and activities for residents to boost general wellbeing & social inclusion.

Eligibility criteria

Inclusions

The main criteria for the WBN is City & Hackney Residents with moderate-severe mental health needs indicated by PHQ4 and people for whom one or more of the following categories are relevant:

  •  Refugees
  • Migrant communities
  • People for whom English is not their first language
  • Residents living in the 10% most deprived neighbourhoods
  • Homeless people
  • People providing at least one hour’s unpaid care and support each week to a friend, neighbour or relative because of illness or old age
  • People with a learning disability
  • Neurodivergent residents
  • Physically disabled people or those with a long-term limiting illness
  • Older adults, especially those who are socially isolated
  • Residents who are digitally excluded
  • LGBTQI+ people
  • Young People
  • African and Caribbean, Turkish and Kurdish, Irish, East and South East Asian, other Middle Eastern, Eastern European and Latin American communities
  • The Charedi Jewish population
  • People experiencing domestic violence
  • People with experience of trauma

Exclusions

Individuals who do not meet criteria will be informed and they can attend open access and safe space events hosted by the Wellbeing Network open to all residents of City & Hackney: https://chwellbeingnetwork.london/
