Rapid Access Clinic is for patients who are deteriorating in the community and are at risk of admission to hospital or long-term care.

Eligibility criteria

Inclusions

Concerned for a patient who needs:

  • urgent attention over the next 24–48 hours to avoid hospital admission or
  • assesment by a consultant who can organise tests and provide urgent advice and a management plan
