The service offers support, information and crisis intervention to patients, families and carers over the telephone and face to face in a patient’s home or usual place of residence. Our Marie Curie nurses can also verify expected death, but the patient must be known to either St Joseph’s Hospice or the Marie Curie Rapid Response team.

This is an overnight rapidly responding service designed for palliative and end of life patients in City and Hackney and Newham. Operating from 10PM-8AM 7days per week, including bank holidays.


Eligibility criteria

Inclusions

  • Wish to stay in their Home/Care Home or are awaiting Hospice admission. Coordinate My Care plan and DNACPR
  • Have a Palliative or Life limiting illness with a ceiling of care to be kept within Primary Care
  • Resident of City & Hackney and Registered with a GP in City & Hackney
  • Over 18

 

Exclusions

Patients who have a preferred place in hospital.

Patients who have a preference for acute / active treatment.

Patients who are medically unstable or have a potentially reversible cause.
