TOENAIL CUTTING PROJECT FROM OCTOBER 2024 FOR 18 MONTHS

(5 appointments at quarterly intervals)

THE DEADLINE FOR APPLICATIONS IS 30 NOVEMBER 2024

Below are the aims of the project:

• To provide an insight to the targeting of future services at those most in need and contribute to a wider strategy for addressing inequalities.
• To understand accessible support to maintain personal care of their toenails whether through training, personal support or access to suitable services
• To support primary care services in the care of their most vulnerable patients and reduce unwarranted attendances into general practice.
• To evaluate the project and outline some options for longer-term solutions to a toenail cutting service for the ICB based on outcomes. Identify the components and benefits of a cross-system and place-based approach.

A successful application is subject to the patient being eligible and agreeing to participate in a pre-appointment and end of project questionnaire.  The questionnaire broadly asks about their medical needs and day to day living and will be done by telephone by Hoxton Health. 

Each GP practice has been allocated a number of patients that can participate in this project.  Please see allocation here for your GP practice for 5 appointments every 4 months over an 18 month period.

Clinic appointments at Hoxton Health, St Leonard’s Hospital

ALL eligibility criteria must be met:

• Registered with a City and Hackney GP
• Aged 75+
• Living alone with no immediate support
• Receiving active care package due to limited physical ability to carry out Activities of Daily Living (A.D.L) including social deprivation.

OR

ALL the eligibility must be met:

• Registered with a City and Hackney GP
• Patient registered on the NHS or Local authority Learning Disability Register
• Living alone with no immediate support

Housebound home visit

• Registered with a City and Hackney GP

The above plus one of the below eligibility criteria must be met:

• Bed bound
• Agoraphobia – NHS psychiatrist/Clinical psychologist has made diagnosis
• On oxygen therapy
• Actively receiving End of Life Care

Hoxton Health also provide:

25 minute appointment* £10.50 will be charged for foot kit on first visit

Toe nail cutting

Thursdays at Hoxton Health 

Hoxton Health are not funded across the borough to do this and charge the full cost of £30.00 plus a one off charge for a reusable foot kit £10.50.

Exclusions

• No serious foot pathology management eg vascular, neurological or surgical issues
• If in doubt about a patient/client the referrer should seek advice from the Homerton Foot Health service Tel: 020 7683 4046 /4347
