New July 2025: City and Hackney Autism Service have recently reviewed and refined their referral process, due to the additional time taken chasing up patient information at the screening stage. We are now requesting that patients give us some additional information at point of referral, including indication for potential ADHD, a questionnaire completed by someone who knew them as a child (RQ1) and the Cambridge Behaviour Scale (EQ40). The information we ask from the GP has remained largely unchanged.
We require all forms to be completed to accept the referral. Where no one is available to complete the RQ1 we can consider the referral but may be unable to reach a diagnostic decision.
