The new Primary Care Quality Alert process is about to launch in City and Hackney. The new Quality Alerts form will be released and available on Monday 21 July in City and Hackney. Other Places will follow as we gradually implement the new process in NEL.
All Practice systems – EMIS and SystmOne – in City and Hackney will have a new primary care quality alert form to enable general practice teams to raise a quality alert, direct with local Trusts or any other provider via the ICB. The new form will auto populate basic information such as practice email and patient NHS number.
Please can City and Hackney practices archive/delete any old form if saved in your favourites.
The Primary Care Portal contains further information including response times, FAQs and the Data Processing Agreement
Quality Alerts – North East London
Please use the new form and emails to raise alerts. Any queries please use the new ICB email for quality alerts
