Launching the C&H Primary Care Type 1 Diabetes Toolkit, to help you refer, signpost and support an adult living with type 1 diabetes (T1DM).
Expected outcomes of the session
To ensure primary care are aware of;
- Services HDC provide for people with Type 1 diabetes
- What pathways HDC has in place for new T1/ existing T1s i.e., pump pathways/ CGM etc
- What primary care need to provide/ signpost patients to with T1 diabetes (prescription essentials, access to services, insulin/ injection checklist, supporting patients who are not engaging with secondary care/ or are high risk)
