GP Attendance at this session will count towards the clinical training requirement for the GP End of Life Care contract.
The event will focus on improving quality of patient / carer experience as well as improving partnership working for end of life care in City & Hackney.
In particular, we will look at:
Improving and embedding patient/carer experience in GP Practice End of Life Care Protocols
Processes and pathways – Care Planning, MARR Charts, DNACPR, Verification of Expected Death
