End of Life Care GP Attendance at this session will count towards the clinical training requirement for the GP End of Life Care contract.
The event will focus on improving quality of patient / carer experience as well as improving partnership working for end of life care in City & Hackney.
In particular, we will look at:
Improving and embedding patient/carer experience in GP Practice End of Life Care Protocols
Processes and pathways – Care Planning, MARR Charts, DNACPR, Verification of Expected Death
