Optimising Diabetes Management in Primary Care: GLP-1 Pathway, DSN & DSD Allocation, and Priority Patient Referrals 

This session addresses key gaps in diabetes management across practices, including underutilisation of DSN support and inconsistent referral practices. Provide support in managing diabetic patients on GLP1s and titrating medications. It aims to streamline care pathways, reduce unnecessary external referrals, and improve outcomes for patients with complex diabetes needs.
