Homerton Healthcare is a well-established North East London (NEL) champion and provider of stroke and neuro rehabilitation services, providing a range of specialist local and regional services across City and Hackney and NEL within the hospital and community. Our stroke, neurology and neurological rehabilitation services serve our patients throughout their lifetime of living with a long-term neurological condition or injury, with patients accessing and re-accessing services as required, ideally, as part of a seamless pathway.

For patients and their carers these services often overlap and smooth transition and communication between services has a significant impact on patient and carer experience as well as their ability to live well with their life changing condition/incident/ disability. Knowledge of the challenges faced by these patient groups, service availability and the ability to input into service change and development is essential to optimise patient experience and outcomes. Homerton has developed the INSPIRE strategy and improvement forum to deliver a coordinated pathway approach to improving the experiences and outcomes for this patient group.
