Advance Care Planning discussions in the Community Setting GPs are often expected to undertake or continue ACP conversations. Whilst this may be with patients well known to you, oftentimes it is a patient/family you’ve had little or no contact with previously.
This session aims to develop communication skills to facilitate Advance Care Planning (ACP) conversations that feel relevant and reassuring to patients and families, taking into account the cultural and religious diversity of the communities we serve.
