The City & Hackney End of Life Care Board is maintaining oversight on the current situation from a palliative care perspective. The key message is as follows:
Hospital services locally are at capacity. When GPs review their patients and identify clinical deterioration that could represent the end of life, please review (or create) a CMC plan. If the patient has identified their preferred place of care as being at home, please consider whether it is more appropriate to trigger a proactive palliative care response rather than an acute admission.
St Joseph’s Hospice and the Marie Curie overnight rapid response team currently have capacity to support palliative care patients at home and the rapid response geriatrician phone line is available to support decision making if needed.
